Meet Theresa DiSipio CPNP-AC, our PANS & PANDAS Expert

Theresa has 15 years of experience in the pediatric oncology specialty where she has worked as a bedside RN, research RN, chemotherapy NP, and clinical educator for hospitals spanning the US and Canada. In her clinical work she has witnessed the impact that acute illness can have on overall wellness, and has seen firsthand the difference that a holistic and root cause approach can have on chronic disease. Theresa is warm, compassionate, and dedicated. She is excited to partner with her patients and their families to create a personalized plan toward healing and optimal health.

Under the leadership of Dr. David Danish, a double board-certified psychiatrist with deep-rooted experience in treating PANS & PANDAS since 2007. 

We are elated to introduce our newest PANS specialist, Theresa Disipio CPNP-AC. Theresa brings her profound expertise as a pediatric nurse practitioner, specializing in PANS, PANDAS, inflammation, chronic medical ailments, and functional medicine.

What are PANS and PANDAS?

PANDAS (Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections) and PANS (Pediatric Acute-onset Neuropsychiatric Syndrome) are terms used to describe a subset of children and adolescents who experience sudden-onset obsessive-compulsive disorder (OCD) and/or tic disorders. PANDAS specifically refers to cases where symptoms worsen following streptococcal infections, such as "Strep throat" or Scarlet Fever. On the other hand, PANS encompasses all cases of abrupt-onset OCD, not limited to streptococcal infections.

What does an intake appointment for PANS vs PANDAS look like?

Intakes can be done virtually or in-person. Theresa Disipio CPNP-AC does the intakes for our PANS & PANDAS clinic. She is an extremely conscientious, thorough, and empathetic provider with years of experience working in Oncology at CHOP, Children’s Hospital of Philadelphia. Prior to the intake, you will talk to an intake coordinator who will walk you through the steps. A family member will complete a comprehensive screening packet that looks at many aspects of the family and patient’s medical, social, and symptom histories. This, along with further information garnered during the intake will be thoroughly covered before interventions such as laboratory studies, nutraceuticals, and/or medication options are discussed.

What does PANDAS look like?

PANDAS and PANS can vary in onset speed, severity, and symptoms. Many cases often show repetitive thoughts, obsessive fears, compulsive actions, and tics. To diagnose PANDAS, several criteria need to be met. Firstly, the individual should exhibit OCD symptoms, tics, or both. Secondly, there must be evidence of a strep culture or a past instance of scarlet fever. If the symptoms have been present for more than a week, a blood test may be necessary. Lastly, there should be an abrupt onset or a significant worsening of symptoms that correlates with the onset of a strep infection.

How do we treat PANDAS?

The primary focus in treating PANDAS and PANS is to tackle the root cause: the strep infection. Often, a single antibiotic course can successfully handle the infection. However, in more intense cases, an extended treatment duration or multiple antibiotic courses might be necessary. Once the core infection is addressed, PANDAS symptoms typically recede. Still, in many cases, recovery can take time and even ebb and flow, and supplementary treatments are often employed to alleviate symptoms during this period.

Dr. Danish graduated from Kenyon College in Gambier, Ohio, and then from Case Western Reserve University School of Medicine in Cleveland, Ohio in 2007, where he earned the Mastin Scholarship for outstanding academic achievement. He came to Philadelphia for his adult psychiatry residency at the Hospital of the University of Pennsylvania, followed by a two-year fellowship in child & adolescent psychiatry at the Children’s Hospital of Philadelphia. He is board certified in both adult psychiatry and child/adolescent psychiatry.During his 15 years of experience in psychiatry, he has treated patients of all ages (from preschool to geriatric) and various backgrounds. He is experienced in treating mood, anxiety, neurodevelopmental, executive function, and psychotic disorders.

After working for over a decade in both emergency room and inpatient settings, gaining experience with the more acute presentations of these disorders, he went on to develop and grow a multi-disciplinary center for psychiatric disorders-- Philadelphia Integrative Psychiatry, located in the suburbs west of Philadelphia, Pennsylvania.

At this practice he leads a team of practitioners, nurse practitioners and therapists, treating hundreds of patients with conditions across the psychiatric spectrum. Philadelphia Integrative Psychiatry employs cutting edge treatment, including the most up to date and research-backed psychopharmacology, neurofeedback, biofeedback, EMDR, and altered mind state for treatment resistant depression (TRD), and soon will provide Transcranial Magnetic Stimulation for TRD and altered mind state assisted therapy for depression & PTSD.

In addition to his role as a clinical lead, he is excited and honored to have been accepted as a member of the team at Behavidence. Behavidence offers a phone-based technology that employs digital phenotyping and a machine-learning algorithm to evaluate onset, progression, and relapse of mental health conditions. At Behavidence he will help direct research, product design, and patient and practitioner outreach and education. This new approach could offer a significant advance in how various conditions are screened for, diagnosed, and monitored in the field of psychiatry.

Juan is a recent graduate of Swarthmore College where they received their BA in Psychology. During their education, they completed research projects exploring Treatment Resistant Depression, Suicidality, PTSD, and effects of Psilocybin Therapy. Juan believes developing a trusting relationship is fundamental for effective therapy and engages with clients' interests and preferred activities to develop positive rapport. As a behavior technician, they aim to help clients gain skills and learn prosocial behaviors.

Dr. Feeney graduated from West Virginia University with masters and doctoral degrees integrating e-learning, counseling, and behavior analysis with Julie Skinner Vargas, daughter of B. F. Skinner.

"Dr. Dave" is a Licensed Behavior Specialist with 25 years' experience helping families with autism, ADD/ADHD, Behavioral Issues, Anxiety, Depression, PTSD, OCD, Weight management, Smoking, and substance use disorders. He is experienced working with children, teens, parents, families, medication management, autism assessment, case management (wraparound), couples and group therapy, and neurofeedback.

For almost 30 years, Dr. Feeney has been a champion of “E-haviorism” - behavior services delivered 100% online.  In 1994, Dr. Feeney founded ONLEARN.COM, a pioneer in online education for lifestyle modification. In 1996, ONLEARN.COM was sold to McGraw-Hill Inc. In 1997, David’s first publication was a book chapter about 100% online smoking control, weight control, and medication compliance services.

Since 1998, Dr. Feeney has held full time leadership positions in higher education, managing digital degree programs at Temple University, Seton Hall University, Strayer University, and more.

Today, Dr. Feeney provides behavior consulting and parent education 100% online. Dr. and Mrs. Feeney are proud parents of an adult son with autism who drives, works 2 jobs, and lives independently.

John Guardiani is a therapist, Vice President and Co-Founder at Philadelphia Integrative Psychiatry. John’s experience in multiple care settings includes - hospital, residential, educational, and outpatient settings. John has helped to promote a diverse and eclectic menu of services at Philadelphia Integrative Psychiatry. With an undergraduate focus in biology and psychology, combined with graduate focus in counseling, John’s mind-body style view of mental health helps drive the underlying integrative mission of the practice.

While employing a warm, patient-centered attitude, John utilizes an inclusive approach to assist clients and their families, helping them to learn more about their bodies and minds, and guiding them to take control of their symptoms. John’s specialties include neurofeedback, biofeedback, and integration of altered mind states with psychotherapy. John also provides expert consultation and services in the management of complex psychiatric cases, including bipolar and psychotic spectrum disorders.

In his leadership role at Philadelphia Integrative Psychiatry, John provides operational and clinical oversight, as well as program development and implementation.
